HDM SLIT may be suitable for children with:
- Allergic rhinitis, causing symptoms such as sneezing, a blocked or runny nose and nasal itching
- Allergic conjunctivitis, causing itchy, watery or puffy eyes
- Allergic asthma that is triggered by house dust mites and is otherwise well controlled
- A confirmed allergy to house dust mites on a skin-prick or blood test
- Persistent symptoms despite appropriate medical treatment
Before considering SLIT, treatments such as steroid nasal sprays, antihistamines and asthma inhalers should be used correctly and optimised. Some families may also consider SLIT because they would like to reduce their child’s long-term reliance on steroid nasal sprays and other symptom-relieving medicines. SLIT does not guarantee that all medication can be stopped. The aim is to improve allergy control and reduce the amount of medication needed where possible.
HDM SLIT involves giving a controlled amount of house dust mite allergen extract under the tongue every day. Regular exposure gradually trains the immune system to become less sensitive to house dust mites. Over time, this can reduce the allergic inflammation responsible for nose, eye and respiratory symptoms.
The first dose is usually given in the clinic under medical observation. Subsequent doses are taken daily at home according to the prescribed treatment plan. Depending on the product used, the dose is placed under the tongue and held there briefly before being swallowed.
HDM SLIT is an effective treatment for appropriately selected children with house dust mite allergic rhinitis or allergic rhinoconjunctivitis.
Many children experience:
Less sneezing, nasal blockage and nasal itching
Fewer itchy or watery eye symptoms
Better sleep and daily comfort
Less reliance on antihistamines and steroid nasal sprays
Better overall control of their respiratory allergies
Children who also have house dust mite-related asthma may experience an improvement in their asthma symptoms. However, SLIT does not replace prescribed asthma preventer or reliever medication unless the doctor advises a change.
Improvement is gradual rather than immediate. Some children begin to notice a reduction in symptoms within the first two to six months, although the response varies between individuals. Completing at least three years of treatment is important, as this increases the likelihood that the benefits will continue after SLIT has been stopped.
HDM SLIT is generally very safe. The most common side effects are mild reactions around the mouth and throat, including:
Itching or tingling inside the mouth or around the lips
Itching of the throat or ears
Mild swelling or discomfort in the mouth
These symptoms usually occur during the early stages of treatment. They are generally temporary and improve as the immune system becomes accustomed to the treatment. If the symptoms are troublesome, your doctor may recommend an antihistamine or adjust the treatment.
Reactions affecting other parts of the body are uncommon. Severe systemic reactions and anaphylaxis are very rare. Families should nevertheless follow the dosing instructions carefully and report any unexpected or persistent symptoms to their doctor.
HDM SLIT requires:
One dose under the tongue every day
Consistent treatment, even when symptoms have improved
Regular follow-up appointments, usually every three months
Continued use of other allergy medicines when needed
Ongoing efforts to reduce house dust mite exposure
During follow-up visits, the doctor will review:
Improvement in nose, eye and respiratory symptoms
Sleep and daily functioning
The amount of other allergy medication required
Side effects and treatment tolerance
Whether any changes to the treatment plan are needed
Daily adherence is essential. Frequently missing doses may reduce the effectiveness of treatment.
The minimum recommended treatment duration is three years. Some children may continue treatment for up to five years, depending on their response, treatment goals and the advice of the allergy specialist.
This is a long-term treatment. Families should consider whether daily dosing and regular follow-up can be maintained before deciding to begin.
Considering HDM SLIT?
A consultation includes reviewing your child’s symptoms, confirming house dust mite allergy and ensuring that current medical treatment has been optimised.
We can then discuss whether HDM SLIT is appropriate and what daily treatment would involve for your family.
